By submitting this form, I verify that the information provided is current and accurate. I understand that all personal and medical information shared will be kept confidential and used solely for the purpose of ensuring a safe and successful experience during my participation in events organized by the Webster Water Ski Collective.
In order to provide a safe and fun experience, my information may be shared with:
Webster Water Ski Collective staff, coaches, or instructors;
Other adaptive sports programs collaborating with the Webster Water Ski Collective;
Medical professionals in the event of an emergency;
Funding agencies or partners, as required for reporting purposes.
I understand that it is my responsibility to inform the Webster Water Ski Collective of any changes to:
My contact information (address, phone number, email);
My medical status, including new diagnoses, surgeries, or other relevant medical updates;
Any other information that may affect the safety of myself or others during participation.
The Webster Water Ski Collective reserves the right to deny participation to any individual if the staff determines that their involvement presents a safety concern to themselves, other participants, or volunteers. This determination may be based on medical, behavioral, environmental, or situational factors.
Furthermore, the Webster Water Ski Collective reserves the right to restrain any participant in the water, in the boat, or on land if such action is deemed necessary to prevent injury to the participant, volunteers, or staff.
